Once a candidate passes the Psychomotor and Cognitive exams, the NREMT will send your National EMT certification in the mail along with a congratulation letter. EMT, AEMT, and Paramedic candidates who successfully complete the NREMT exam in Oregon are also awarded their Oregon State Certification.
This also applies to EMT Intermediates. An EMT-I will receive Oregon State Licensure automatically after successfully passing the Oregon State exam.
Oregon EMT License
The Oregon EMT State license is given to applicants at the same time they are given their National license through the NREMT. Except for EMT-I who are given an Oregon State EMT-I License when they complete an EMT training program and pass the Oregon state EMT-I test.
Candidates in Oregon may be asked to provide personal information while applying for an Oregon State EMT license. Applicants should be prepared to answer questions about topics like drug use, criminal history, etc. Below are examples of questions likely to be found on the Oregon EMT license application form.
• Have you ever resigned or surrendered a healthcare license or certificate?
• Have you had within the past 10 years any mental or physical condition that impaired or could impair your ability to perform the duties required of an EMS Provider? If yes explain and is condition still present?
• Do you have any mental or physical condition that impairs or could impair your ability to perform the duties required of an EMS Provider? If yes, explain whether the condition is controlled by medication or other treatment.
Also explain how your condition treated or untreated can affect your ability to perform the duties of an EMS Provider.
• Have you ever been arrested, charged with, or convicted of any felony or misdemeanors?
• Do you currently use or have you used in the last ten years any chemical substances or drugs other than for legitimate medical purposes?
This includes drugs that impair or has impaired your ability to perform the duties required of an EMS Provider in Oregon?
• Within the last ten years have you been counseled about, diagnosed with, or treated for a dependency on alcohol or drugs?
Applicants should be aware that the following questions can be asked on the National Registry and/or the Oregon Application for licensure.
• Has an employer or supervising physician taken disciplinary action against you related to your duties as an EMS Provider? (Discipline includes suspension, letter of reprimand, resignation in lieu of termination, a limitation or restriction of scope of practice or dismissal for cause.)
• Have you been named in a lawsuit alleging medical malpractice or misconduct related to providing medical care?
• Have you ever been disciplined, denied or revoked by the National Registry of EMTs or any health care certifying/licensing agency?
Oregon Scope of Practice
The EMT scope of practice is a set of EMS protocols that guide Emergency Medical Responders in Oregon. The EMT scope of practice helps all levels of emergency response personnel to manage prehospital situations and provide the correct emergency medical care and treatments for patients.
In Oregon the Oregon Medical Board is responsible for governing the scope of practice for all Emergency Medical Service Providers.
Oregon EMR Scope of Practice
An Oregon Emergency Medical Responder (EMR) can perform the following:
1. Primary and Secondary patient assessments.
2. Take and record vital signs.
3. Administer oxygen.
4. Use a bag mask ventilation device with reservoir.
5. Provide hemorrhage control.
6. Assist with prehospital childbirth.
7. Complete accurate prehospital emergency care reports with all patient contacts. EMR must provide a copy of reports to senior EMS provider with the transporting ambulance.
8. Position patient’s head to open and maintain an airway.
9. Maintain an open airway using: A) Nasopharyngeal device, B) Non-cuffed oropharyngeal airway, and C) Pharyngeal suctioning device.
10. Care for musculoskeletal injuries.
11. Perform external Cardiopulmonary Resuscitation (CPR) and obstructed airway care for adults, children, and infants.
12.Provide emergency transport for at risk patients.
13. Use noninvasive diagnostic devices when appropriate.
14. Prepare and administer aspirin by mouth for suspected Myocardial Infarction (MI). Patient must have no recent GI bleed and no allergy to aspirin.
15. Provide care for patients with suspected medical emergencies.
16. Administer liquid oral glucose for hypoglycemia.
17. Prepare and administer naloxone via auto-injector or intranasal device for suspected opioid overdose.
18. Prepare and administer epinephrine via automatic injection device for anaphylaxis.
19. Perform cardiac defibrillation with a semi-automatic or automatic defibrillator if the Emergency Medical Provider has successfully completed an Authority approved course as required.
20. EMRs can perform other emergency tasks if ordered by a physician as long as they are done under direct visual supervision of that physician.
Oregon EMT Scope of Practice
An Oregon EMT can perform all the above procedures that an EMR can do along with the following:
1. Insert a supraglottic airway device.
2. Provide care for patients in shock.
3. Ventilate with a non-invasive positive pressure delivery device.
4. Perform tracheobronchial tube suctioning.
5. Provide care for patients with suspected medical emergencies.
6. Administer activated charcoal for suspected poisonings.
7. Obtain capillary blood sample for blood glucose monitoring.
8. Prepare and administer albuterol treatments for known chronic obstructive pulmonary disease (COPD) and asthma patients experiencing bronchospasm.
9. Prepare and administer epinephrine by subcutaneous injection, intramuscular injection or automatic injection device for anaphylaxis.
10. Assist patient with administration of sublingual nitroglycerine spray, tablet, or metered dose inhaler that patient is in possession of. Nitroglycerine must also be prescribed to patient by their physician.
11. Complete a clear and accurate prehospital emergency care report with every patient contact.
12. Transport stable patients with in-dwelling vascular devices, saline locks, heparin locks, and foley catheters.
13. Perform cardiac defibrillation with a semi-automatic or automatic defibrillator.
14. Assist AEMT, EMT-I, or Paramedics on scene by: A) Assembling and priming IV fluid administration sets B) Assembling and uncapping preloaded medication syringes and vials.
15. In a declared Mass Casualty Incident (MCI) can monitor patients with isotonic intravenous fluids flowing.
16. An Oregon EMT who has completed Authority training can participate in the event of a release of organophosphate agents. The EMT can prepare and administer atropine sulfate and pralidoxime chloride by autoinjector.
Oregon Advanced EMT Scope of Practice
An Oregon AEMT can perform all the above procedure that an EMT can perform along with the following:
1. Initiate saline locks.
2. Initiate and maintain peripheral intravenous lines.
3. Obtain peripheral venous blood specimens.
4. Initiate and maintain an intraosseous infusion in pediatric patients.
5. Prepare and administer the following medications under direct orders from a physician:
A) Naloxone
B) Glucagon
C) Albuterol
D) Hypertonic Glucose
E) Anti-hypoglycemics
F) Nebulized Bronchodilators
G) Ipratropium Bromide
H) Physiologic Isotonic Crystalloid Solution
I) Epinephrine for anaphylaxis
J) Nitrous Oxide as an analgesic for acute pain
K) Lidocaine as an anesthetic intraosseous infusion
L) Nitroglycerine as a vasodilator.
Oregon EMT Intermediate Scope of Practice
An Oregon EMT-I can perform all procedures above that and Advanced EMT can perform along with the following:
1. Insert an Orogastric tube.
2. Perform electrocardiographic rhythm interpretation.
3. Perform cardiac defibrillation with a manual defibrillator.
4. Prepare and administer the following medications under direct orders from a physician:
A) Anti-emetic: a)Ondansetron
B) Vasoconstrictors: a)Epinephrine b)Vasopressin
C) Diuretic: a)Furosemide
D) Antiarrhythmics: a)Atropine Sulfate b)Lidocaine c)Amiodarone
E) Antihistamine: a)Diphenhydramine
F) Analgesics for acute pain: a)Morphine b)Nalbuphine Hydrochloride b)Ketorolac Tromethamine d)Fentanyl
5. In the event of a mass distribution effort, an Oregon EMT-Intermediate can distribute medications at the direction of the Oregon State Public Health Officer.
6. In the event of an epidemic or outbreak, an Oregon EMT-I can prepare and administer immunizations as part of an emergency immunization program.
7. Prepare and administer immunization for pandemic and seasonal influenza vaccinations.
8. Prepare and administer emergency or routine immunizations and tuberculosis skin (TB) testing.
9. Maintain intravenous medication infusions and other procedures during transport if they were initiated in a medical facility with written and verbal orders.
Oregon Paramedic Scope of Practice
An Oregon Paramedic can perform all the above EMT-Intermediate procedures along with the following:
1. Perform electrocardiographic interpretation.
2. Initiate a nasogastric tube.
3. Initiate and maintain urinary catheters.
4. Access central intravenous ports and indwelling catheters for fluid and medication administration.
5. Obtain peripheral arterial blood specimens.
6. Initiate a needle thoracotomy for tension pneumothorax in the prehospital setting.
7. If formally trained on the equipment, an Oregon paramedic can initiate and maintain mechanical ventilation during transport.
8. Initiate the following airway management techniques:
A) Endotracheal Intubation
B) Cricothyrotomy
C) If no other mechanism is available to establish an airway Transtracheal Jet Insufflation can be used.
9. Provide advanced life support (ALS) for resuscitation of patients in cardiac arrest.
10. Perform emergency cardioversion.
11. Attempt external transcutaneous pacing in a bradycardia patient with hemodynamic compromise.
12. Prepare and administer any blood products or medications within written protocols when authorized under direct orders of a physician.
